Using Insurance Benefits

If you plan to use insurance, you should contact your insurer to verify your benefits before our first appointment.  Questions you may want to ask your insurer include:

  • What are my mental health benefits?
  • Is the provider I wish to see covered by my plan?
  • Do I need pre-authorization to see a mental health provider?
  • Do I need to have a referral from my primary care provider?
  • Do I have a deductible to satisfy?
  • Do I have a copay?
  • Are there a limited number of sessions per year on my plan?

I am a preferred provider for the following insurance plans:  Premera Blue Cross, Regence Blue Shield, First Choice and Uniform Medical Plan.  If I am not an in-network provider for your particular insurance plan, you may be able to see me as an out-of-network provider (please call your insurer to find out whether they will reimburse for services provided by an out-of network provider). If you need me to contact your insurer for pre-authorization, please let me know in advance of your first appointment.  Please bring your insurance card to our first appointment, if you plan to use your insurance for payment.
